Charge Conference Forms Breakdown
From the District Secretary you will find our district administrative assistant's Guidelines and Reminders
Form 101 & Form 102 - Local Church and Charge Officers - Both of these forms must be filled in and accurate. Our district administrative assistant will use these forms to update your officers next year in her files. When available email addresses for all of your key officers are needed.
Form 103 is no longer in hard copy. The Nominations Profile Form is now found on and needs to be completed on line at the conference web page at www.vaumc.org. This form is used for nominating any one in your church that has the desire and ability to serve on a district or conference committee. We use these names as a pool of qualified people in which we make nominations for our district and conference slate of officers each year.
Form 104, Form 105, Form 106, and Form 107 – These forms need to be filled in by the appropriate officers. These reports are listed on your Conference Form 1 – Minutes of the Charge/Church Conference.
Form 108 & Form 109 are the Parsonage Review and Inventory Forms. Please save these forms to use next year. These forms are for your use in reviewing the parsonage. They can be completed anytime after the first of the year but they need to be returned to the district office by June 1.
Form 110 - Trustees Form – This is for your use when reporting or changing your Trustees with the court. It is just a guideline if needed. Each county has their own set of rules to go by. Check with your Circuit Court to see what they require before beginning the process. Trustees should be re-registered whenever there is a change.
